Neurodiversity Referrals
Children under 18 years
If you are concerned, you should speak to your child’s school about arranging a referral for autism or ADHD. Only request a referral via the GP surgery if this has not been possible.
To refer a child aged 2.5 – 7 years we need:
- CDS referral form for the Child Development Service
To refer a child aged 7 – 18 years old for an assessment in Bradford by CAMHS we need:
- One SNAP form from home and one from school
- Completed development history by parents/guardians
- Completed observations from school
The forms that you require can be downloaded from Bradford Schools Online , please hand these forms into reception once completed. There is a long wait for an autism or ADHD assessment.

Adults
- Contact reception to ask for copies of our Adult Autism and ADHD questionnaires
- Make a GP appointment to discuss next steps after questionnaires have been completed
Right to choose for Adults and Children
This is your Right to Choose where you or your child receives NHS treatments and assessments. Please be aware that recommendations for medication may not be actioned by your GP as some of these providers do not have an NHS medication agreement in place. Most patients do not need medication. It may be recommended for people with ADHD. If an assessment is carried out and medication is recommended by a private provider we may need to refer you to an NHS service.

NHS West Yorkshire Integrated Care Board (WYICB) have recently completed an accreditation process of providers of ASD and ADHD assessment and treatment service for Right to Choose. If you wish to choose a different provider this means WYICB does not have a direct contractual relationship with them. Before a referral is made the GP must seek confirmation from the WYICB Contracting Team that the provider has a current qualifying NHS Standard Contract for the age of the individual for that service.
The list above changes frequently so please enquire with the practice if you wish to use another provider.
