Before filling out a registration form, check you are in our catchment area. Our practice is registering new patients from the following postcodes: BD2, BD3, BD10 and part of BD1
Please consider registering with Bevan House 01274 322400 for specialist GP support if you are:
- Homeless
- Refugee
- Asylum Seeker
Register with Farrow Medical Practice
- Complete the online form below
- If you are unable to complete the form and would like to register with our practice please speak to one of our receptionists
- If you have a long term condition or are on regular medication we may contact you to attend for a new patient health check
- Registration can take up to 48 hours
- If you require a doctor or nurse in the meantime please contact your previous GP or NHS 111

Online Access to Medical Records
We are able to offer online access to medical records for most patients. Proof of ID and registration is required for this service and you will be given a user name and password that must be kept secured. We advise you to discuss this with our reception team.
Setting up an NHS account
To set up your NHS account for the first time, you will need to set up an NHS login. Read about how to set up NHS login (nhs.uk).
Once you have set up your NHS login, you can log in to your NHS account or download the NHS App onto your phone.
You can use your NHS Account or the NHS App to:
- order repeat prescriptions
- view your test results
- see parts of your health record
- check appointments
- see your vaccination record
Accessing someone else’s information
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.
To request proxy access:
- collect a proxy access form from reception from 10am to 6pm
